Physical Therapist (PT) Home Care - Per Diem

Job Summary
Evaluates patient conditions and develops individualized physical therapy plans of care to restore function and prevent disability. Implements evidence-based treatments including therapeutic exercises, manual techniques, and functional mobility training while facilitating patient education and independence. Provides reassessments to modify plans and communicates progress to physicians and supervisors. Delivers cost-effective care consistent with departmental billing guidelines and completes all required documentation per regulatory standards. Requires a licensed physical therapist with ability to travel to various home care locations, lift 40-50 lbs, and work variable day shifts of minimum 24 hours monthly.
Required Qualifications
- Bachelor's Degree Physical Therapy
- Licensed Physical Therapist - State of Pennsylvania
- American Heart Association Basic Life Support
- Ability to work at various locations per diem
- Knowledge of best practices in physical therapy
- Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses
- Must be able to provide safe, compassionate, and competent physical therapy treatments
- Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner
- Ability to travel to patient care assignments - Home Care
- Lift and carry 40-50 lbs

Desired Qualifications
- Master's Degree Physical Therapy or Doctorate Physical Therapy
- 1 year clinical experience for Home Care
