Manage patient care and documentation support in home health environments across Eden Prairie, Apple Valley, Bloomington, Eagan, Wayzata, Maple Grove, Elk River, Anoka, Woodbury, Stillwater, Hugo, and Blaine. Deliver exceptional clinical services directly to patients' homes while handling verbal orders, chart preparation, and administrative tasks to reduce paperwork burdens. Maintain full-time availability on Monday through Friday during day shifts with flexible scheduling options, including four bonus on-call Saturdays annually. Hold a current Minnesota Physical Therapist license, valid CPR certification for Health Care Providers, and a valid driver's license. Required Qualifications
