Job Summary
Conduct comprehensive initial and ongoing assessments for patients, evaluating status changes since the last visit to determine necessary service modifications. Deliver customized home-based therapy to help individuals regain strength, manage pain, and achieve personal goals. Submit documentation of skilled interventions and participate in educating patients, families, and caregivers to promote progress toward established objectives. Requires current Arizona licensure, one year of Physical Therapy experience, and CPR certification. Full-time or part-time roles available with competitive weekly pay, travel time reimbursement, and a signing bonus.
Required Qualifications
- Current licensure or certification as a Physical Therapist active in Arizona
- One (1) year Physical Therapy experience
- CPR Certification (demonstration course required)
- Valid Arizona Fingerprint Clearance card
Desired Qualifications
- Experience working in home care setting preferred
