Job Summary
Home Health Physical Therapist performs in-home assessments and delivers individualized therapy to improve strength and mobility. Responsibilities include assessing patient status over time, modifying plans as needed, documenting treatment and skilled interventions, and educating patients and families/caregivers to meet established goals. Requires active Arizona Physical Therapist licensure, 1 year PT experience, CPR, and Arizona Fingerprint Clearance; home-health setting and travel are part of the role with mileage/travel-time reimbursement; 1:1 patient ratio and continuing education benefits are provided.
Required Qualifications
- Current licensure or certification as a Physical Therapist active in Arizona
- One (1) year Physical Therapy experience
- CPR Certification
- Valid Arizona Fingerprint Clearance card
- No Covid Vaccination Required
