Job Summary
Evaluate and treat patients in a post-acute/skilled nursing setting, developing individualized treatment plans focused on improving mobility, strength, balance, and independence. Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and families to help patients achieve meaningful functional outcomes. Educate patients and caregivers on treatment plans and home exercise programs while completing accurate, timely documentation.
