Job Summary
Conduct comprehensive initial and ongoing assessments at appropriate intervals, evaluating changes in patient status since the last visit. Determine necessary modifications to planned services and submit documentation of all treatment and skilled interventions provided. Educate patients, families, and caregivers to promote progress toward mutually established goals while maintaining a 1:1 patient ratio. Requires current Arizona licensure, one year of physical therapy experience, and CPR certification. Full-time role with weekly pay, travel time reimbursement, and a $1000 signing bonus available until September 30, 2026.
Required Qualifications
- Current licensure or certification as a Physical Therapist active in Arizona
- One (1) year Physical Therapy experience
- CPR Certification (demonstration course required)
- Valid Arizona Fingerprint Clearance card
Desired Qualifications
- Experience working in home care setting preferred
