Job Summary
Travel to patients' homes to provide physical therapy as ordered by a physician, following the designated plan of care. Design personalized treatment plans and evaluate treatment effects to adjust interventions for maximum benefit. Diagnose movement dysfunction through patient observation and consultation regarding medical history and symptoms. Deliver hands-on therapy such as massage to ease pain and promote healing while maintaining detailed patient records. Instruct family members and caregivers in appropriate techniques and recovery expectations. Requires current NY State licensure, accredited program graduation, and proven clinical expertise.
Required Qualifications
- Current Physical Therapy Licensure in NY State
- Graduate of an accredited PT program
- Proven work experience as a physical therapist
- Intensive education and clinical expertise
- Current knowledge of treatment practices
- Excellent interpersonal communication skills
