Job Summary
Physical Therapist to perform initial and ongoing assessments, develop plan of care, and deliver physical therapy services in home health across Minnesota branch territory. Responsibilities include coordinating with healthcare team, supervising PTA/HHA as applicable, completing OASIS assessments where allowed, maintaining documentation, and discharge planning with patient and caregivers. Requires licensure in state of practice and a Degree in Physical Therapy; opportunities include benefits such as 401(k), health insurance, PTO, DEI program, and tuition reimbursement.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR, Drivers License and Automobile Insurance
