Physical Therapist (PT)

Physical Therapist (PT) to provide in-home skilled therapy services across patients' homes in Weber, Davis, Salt Lake, and Utah counties. Responsibilities include evaluating patient function, developing and updating individualized care plans, delivering treatments using appropriate modalities, monitoring progress and communicating with the physician and care team, educating patients and caregivers on goals and techniques, reassessing needs, collaborating with interdisciplinary team, participating in quality improvement, staff meetings, ongoing education, and supervising the Physical Therapy Assistant at least every 30 days. Requires driving to patient homes, ability to lift/move equipment, and regular tablet-based documentation. Licensed in Indiana and graduate of an accredited PT program with at least 1 year of PT experience are required.
