Job Summary
Deliver physical therapy to homebound patients of all ages in Stockton and surrounding areas using the Cliniconnects app to accept or decline referrals. Observe, record, and report patient responses to treatment, then determine and execute the individual plan of care. Complete documentation during visits via our intuitive electronic medical record system while managing your own schedule and taking time off as an independent contractor. Earn per-visit rates based on the number of patients you treat. Current, unrestricted state licensure and a smartphone are required.
Required Qualifications
- Ability and enthusiasm in working with all ages
- Current Physical Therapist license issued by the state Board of Physical Therapy
- The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
- Completion of an accredited Physical Therapy program
- Reliable personal transportation
- Smartphone
