Job Summary
Deliver one-on-one physical therapy to older adults in private residences, senior living communities, and via telehealth to improve mobility and functional independence. Document care effectively within the FOX EMR system while collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Assist patients with transfers and bed mobility, driving to patient locations as part of the Geriatric House Calls model. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to meet the physical demands of in-home and community-based care, including: Driving to patient homes and senior living communities as part of the Geriatric House Calls model
- Must be available to work in Bridgeton, NJ, and surrounding communities
Desired Qualifications
- Experience with telehealth and other forms of virtual care
