Job Summary
Provide in-home physical therapy to Texas home health patients. Manage an independent caseload, conduct comprehensive in-home assessments, and deliver skilled PT visits. Develop and execute a clear plan of care focusing on gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention. Drive multidisciplinary coordination, track outcomes, and adjust treatment as needed. Supervise PTAs where appropriate and coach patients and caregivers with practical home programs. Document promptly in Homecare Homebase (HCHB). Role is field-based with travel across the assigned Texas territory; schedule is full-time with Monday–Friday plus occasional weekends, leveraging technology to support efficiency.
Required Qualifications
- Active Texas PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PT scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
