Job Summary
Licensed Physical Therapist to evaluate, plan, and deliver individualized physical therapy services to skilled nursing residents; supervise PTAs and rehabilitation aides; develop and implement plans of care with measurable functional goals; deliver both individual and group therapeutic interventions to restore function; monitor progress and adjust treatments; participate in MDS assessments, care conferences, discharge planning, and quality-improvement initiatives; ensure compliance with HIPAA, OSHA, and state practice standards; provide home exercise programs and caregiver instruction; communicate findings to physicians, team, patients, and families; maintain compliant documentation and professional leadership in a geriatric/post-acute care setting; role is non-exempt and reports to Rehabilitation leadership and Facility Administrator; strong clinical judgment, organization, and teamwork are essential.
Required Qualifications
- Current, active, and unrestricted Physical Therapist license in the State of California — required
- Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate) — required
- Successful completion of the National Physical Therapy Examination (NPTE) — required
- Current CPR/BLS certification (or willingness to obtain per facility policy) — required
