Job Summary
Assess patient needs, develop recovery plans, and deliver physical therapy to help older adults stay active and children thrive in home or school settings. Design individualized programs that restore, reinforce, and enhance physical abilities while training patients and caregivers on skills for independence. Collaborate with peers to make referrals when necessary, adapting care to diverse patient needs across skilled nursing centers, assisted living facilities, outpatient clinics, and home-based care. Work in a facility environment reporting to the Director of Rehab, with a focus on patient-centered recovery and personalized wellness solutions.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
