Job Summary
Evaluate patients within 72 hours of receiving a physician referral and develop individualized Plans of Care. Provide skilled therapy services to home health patients, teach family members proper care techniques, and supervise PTAs and HHAs. Communicate effectively with physicians and the interdisciplinary team while maintaining accurate documentation in WellSky (Kinnser) EMR. Update care plans based on treatment effectiveness and participate in team meetings and continuing education. Maintain patient confidentiality per HIPAA standards. Available Monday–Friday, 8 AM–5 PM.
Required Qualifications
- Active Physical Therapist (PT) license
- Minimum 1 year of PT experience
- Physical (within 12 months of first patient contact)
- Current TB test (within last 12 months)
- Current CPR certification
- Valid driver's license & reliable transportation
- Must be available to attend orientation at our Willowbrook office
- Available Monday–Friday, 8 AM–5 PM
Desired Qualifications
- WellSky (Kinnser) EMR experience a plus
