Physical Therapist (PT)

Review patient charts and medical history, develop personalized treatment plans, and teach therapeutic exercises to improve patient conditions. Use hands-on therapy to promote healing while maintaining accurate records by tracking goals and adapting plans accordingly. Work closely with a supportive team including physicians, nursing, social services, and home health aides to provide care to the highest standard. Deliver patient and family education for health promotion as directed by the physician's plan of treatment. Requires a New York State PT license, one year of recent hands-on experience, and a valid driver's license. Schedule includes five 8-hour shifts (8:30 AM to 4:30 PM) with rotating weekends.
