Physical Therapist (PT)

Provide direct patient care as a Physical Therapist in a Skilled Nursing Facility setting, developing individualized therapy programs through physical assessment, planning, implementation, and education to maximize patient progress toward functional goals. Collaborates with other health professionals, documents evaluations and treatment goals, maintains up-to-date documentation and prescriptions, and participates in ongoing professional development while adhering to standards of care and infection control. Must possess a state Physical Therapy license and a doctoral, masters, or bachelor’s degree in Physical Therapy; travel as needed.
