Physical Therapist (PT)

Evaluate residents' functional abilities and develop individualized treatment plans to address physical impairments and improve quality of life. Provide direct physical therapy interventions, including therapeutic exercise, gait training, balance activities, and pain management. Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility, state, and federal requirements. Collaborate with an interdisciplinary team including physicians, nurses, occupational therapists, speech-language pathologists, and social workers to coordinate resident care. Educate residents, families, and caregivers on therapy programs, safety techniques, and strategies to maximize independence. Monitor residents' progress and adjust treatment plans as needed while adhering to ethical standards and infection control protocols. Participate in care plan meetings and quality improvement initiatives.
