Physical Therapist (PT)

Provides and directs Physical Therapy services to residents in a skilled nursing and rehabilitation center, including assessment, treatment planning and implementation, related documentation, and communication; functions under physicians' orders. Responsibilities include conducting screenings, evaluating residents for intervention, performing specialized evaluations, maintaining confidentiality, implementing treatment as outlined, following management direction, and performing additional duties as assigned. Requires a Bachelor of Science in Physical Therapy, successful completion of the National Certification Examination for Registered Physical Therapist, current state licensure or licensure eligibility, and 0-3 years of long-term care experience. Background screening through AHCA Care Provider Clearinghouse is required.
