Job Summary
Physical Therapist (PT) needed to provide pediatric home-based therapy across the community. Responsibilities include evaluating and interpreting assessment results, developing and implementing individualized PT treatment plans for children with physical, neurological, cognitive, and social/emotional needs; empowering families to carry over therapeutic activities in daily routines; coordinating with referral partners; instructing patients and caregivers on home exercise programs; and documenting care in an EMR. Requires a state license and CPR certification; 1 year of experience preferred.
