Physical Therapist (PT)

Physical Therapist (PT) to implement pediatric in-home and community-based therapy programs. Responsibilities include evaluating and interpreting assessment results, developing goals for each child, creating and implementing individualized treatment plans, assisting pediatric patients to improve or regain physical, neurological, cognitive, and social-emotional functioning, educating and empowering caregivers to carry over therapies at home, coordinating with referral partners, ensuring continuation of therapeutic plans after discharge through home exercise programs, documenting care and care coordination in an EMR, and maintaining patient confidentiality.
