Job Summary

Educate and empower caregivers to carry over therapeutic activities at home, coordinate with referral partners, ensure continuity of care with home exercise programs, and document patient care in an EMR. Requires state license and CPR certification; opportunity offers part-time scheduling with a comprehensive benefits package.
