Job Summary
Physical Therapist (PT) to implement treatment programs for pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays in home and community settings. Responsibilities include evaluating patients, developing goals, creating and implementing PT treatment plans with physicians, helping children improve functioning, educating and empowering caregivers to carry over activities, coordinating with referral partners, designing home exercise programs for post-discharge, and documenting care in an EMR system while maintaining patient confidentiality.
