Job Summary
Certified Physical Therapist (PT) to implement pediatric home-health PT programs in home and community settings. Responsibilities include evaluating children, developing goals, creating and implementing PT plans with physician, educating caregivers to integrate therapy into daily routines, coordinating with referral partners, and ensuring continuity of care post-discharge through home exercise programs. Maintain patient confidentiality and accurate EMR documentation. Required Qualifications
- Texas State PT license
