Job Summary
Provide pediatric physical therapy in home and community settings to children with physical, neurological, cognitive, and social-emotional disabilities; develop and implement goals and treatment plans, evaluate progress, and adjust plans in collaboration with physicians; educate, coach, and empower caregivers to integrate therapeutic activities into daily routines; coordinate with referral partners and discharge planning; design and instruct home exercise programs; maintain patient documentation in an electronic medical record system while preserving confidentiality.
