Job Summary
Conduct comprehensive patient evaluations and provide skilled physical or occupational therapy services in the home setting. Develop individualized plans of care based on each patient's condition, needs, and goals while monitoring progress to modify treatment plans as clinically appropriate. Complete timely and accurate visit documentation and educate patients, family members, and caregivers regarding treatment plans, safety, mobility, and home exercises. Collaborate with physicians, nurses, and interdisciplinary team members to communicate changes in patient conditions, ensuring all services and documentation comply with agency policies and applicable state and federal regulations. Requires an active and unrestricted Florida license, minimum six months of experience, and ability to travel within the assigned service area.
Required Qualifications
- Active and unrestricted Florida Physical Therapist license
- Minimum of six months of physical therapy experience
- Strong patient assessment, treatment planning, documentation, and communication skills
- Active and unrestricted Florida Occupational Therapist license
- Minimum of six months of occupational therapy experience
- Strong patient assessment, treatment planning, documentation, and communication skills
- Valid driver's license
- Reliable transportation and current automobile insurance
- Ability to successfully complete an AHCA Level 2 background screening
- Ability to travel to patients' homes within the assigned service area
- Excellent organizational and time-management skills
- Ability to work independently while collaborating with an interdisciplinary care team
Desired Qualifications
- Home health experience preferred
- Current CPR or Basic Life Support certification preferred
