Job Summary

Provide skilled physical therapy in patient homes as ordered by attending physicians, conducting initial evaluations within 48-72 hours of referral and developing individualized plans of care. Educate patients and families on disease management, equipment use, and preventative measures to decrease re-hospitalizations, while ensuring safety and accurate clinical documentation in accordance with Medicare and federal guidelines. Supervise therapy services provided by PTAs and nurse assistants, adjusting plans based on patient response and completing chart audits as requested. Drive to assigned locations daily, participate in on-call rotations, and maintain active licensure.

Required Qualifications

  • Physical Therapist (PT) with a current license in the state of employment
  • Minimum of one year of experience as a Physical Therapist (PT) in an acute setting
  • Valid driver's license and auto insurance with your name as a listed driver
  • Must be able to meet all physical position requirements
  • Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation
  • Annual tuberculin skin test requirement thereafter unless documentation of previous TB test exists
  • Must be free of physical/medical conditions which would limit or restrict ability to perform job functions
  • Must be able to drive to patient's primary location per scheduled visit
  • Must be able to lift up to 50 lbs. with or without assistance
  • Must be able to stretch/reach
  • Must be able to walk
  • Must be able to sit
  • Must be able to bend/stand
  • Must be able to climb
  • Must be able to stoop (bend at waist)
  • Must be able to read, speak, and write English
  • Must be able to use computer/PDA

  • Must be able to distinguish smell/taste occasionally
  • Must be able to travel for this position
  • Must be able to participate in all on-call requirements
  • Must be able to attend in-service trainings and mandatory agency meetings
  • Must be able to maintain a professional appearance
  • Must be able to understand and practice agency policies and procedures
  • Must be able to complete initial evaluation and admission visit within 48-72 hours of referral
  • Must be able to complete chart audits at agency site location as requested
  • Must be able to complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
  • Must be able to complete all clinical documentation within specified company requirements
  • Must be able to establish and follow plan of care as permitted within scope of practice
  • Must be able to communicate with scheduler any changes as needed
  • Must be able to participate in case conferences
  • Must be able to participate in orientation/training of new employees
