Physical Therapist (PT)

Provide home-based physical therapy services for patients as ordered by the attending physician across the Huber Heights, Dayton, Fairborn, and Yellow Springs areas. Responsibilities include performing initial evaluations, developing/update plans of care, administering therapeutic interventions (e.g., exercises, manual therapy, modalities), educating patients and families, performing OASIS assessments as needed, documenting all care in accordance with agency and Medicare guidelines, supervising PTAs and aides, coordinating with the interdisciplinary team, and ensuring continuity of care. Requires maintaining an active PT license, driving to patient homes, and complying with regulatory and agency requirements. Ability to communicate effectively with patients, families, physicians, and coworkers; strong attention to detail; and willingness to participate in on-call and case conferences. A $20,000 bonus is offered and mileage is reimbursed; schedule is Monday–Friday, 8:30 am–5:00 pm.
