Job Summary
Conduct comprehensive in-home physical therapy assessments and deliver skilled visits to manage a full caseload of approximately 28 productivity points weekly. Build and execute clear plans of care focusing on gait, balance, transfers, and home safety while coaching patients and caregivers on practical home programs. Drive multidisciplinary coordination by communicating condition changes, aligning on orders, and supervising PTAs as appropriate. Document thoroughly in Homecare Homebase and participate in case conferences to maintain low hospitalization rates and high patient-experience scores. Maintain a full Michigan PT license and operate independently across a Detroit territory, utilizing software to accelerate charting and provide feedback on platform improvements.
Required Qualifications
- Active Michigan PT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver's license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health PT scope
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
- Strengthening, functional mobility, and home safety assessment/modification education
- Patient/caregiver coaching with practical home programs
Desired Qualifications
- home health experience strongly preferred
- Prior home health experience
- Homecare Homebase (HCHB) experience (or similar home health EMR)
- Deeper clinical experience in high-signal PT populations
- Experience supervising/mentoring PTAs and collaborating on care plans
- Strong organization and time management
