Job Summary
Coordinate with clinical caregivers to ensure residents and patients achieve full recovery and timely return to activities under the integrated care model. Provide skilled rehabilitation services to residents in Signature HealthCARE nursing homes nationwide and on an outpatient basis. Maintain a valid Physical Therapy license in the state where services are rendered and hold a minimum Bachelor's of Science degree from an accredited program. Join a team fostering person-directed care within a facility receiving high performance ratings and Five-Star CMS scores.
Required Qualifications
