Job Summary
Provide in-home physical therapy services under a physician’s plan of care, evaluating patients’ rehabilitation needs and developing/directing PT programs focused on functional recovery. Supervise PTAs/STNAs, collaborate with the care team, and participate in discharge planning. Educate patients, families, and caregivers in the use of therapeutic devices; document progress in clinical summaries; and coordinate care with nursing staff. Requires active Ohio PT license, CPR, and ability to travel to patient homes; role includes on-the-road visits with some remote work from the local office. Ideal candidates have 2+ years of PT experience, home health or community-based care background, strong communication, empathy, and organizational skills; benefits include health/dental/vision, 401(k) with matching, PTO, mileage reimbursement, and referral program.
Required Qualifications
- Active Ohio PT license
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
- CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Recent TB test
