Job Summary
Deliver personalized therapy programs for personal care residents, including assessment, treatment planning, implementation, and related documentation. Integrate interactive and gamified tools into care plans while supporting patients with neuro conditions and cognitive decline. Contribute to the development and education of screening processes within the new tech-forward neurorehab program. Functions under a physician's order with the ability to make independent decisions when circumstances warrant. Requires a current, valid Physical Therapist license and BLS certification from a Vibra-approved vendor. One year of experience and knowledge of Medicare skilled services requirements are preferred.
Required Qualifications
- Current, valid, and active license to practice as a Physical Therapist in the state of employment
- Current BLS certification from a Vibra-approved vendor
- Freedom from illegal use of and effects of use of drugs and alcohol in the workplace
Desired Qualifications
- Knowledge of One (1) year of experience preferred
- Skilled Nursing Facility Medicare skilled services requirements preferred
- Ability to project a professional image
- Knowledge of regulatory standards and compliance requirements
- Strong organizational, prioritizing and analytical skills
- Ability to make independent decisions when circumstances warrant
- Working knowledge of computer and software applications used in job functions
