Physical Therapist - PRN

Conduct home health physical therapy assessments and evaluations of patient functional status, mobility, endurance, and pain. Develop individualized treatment plans and perform interventions to improve patient well-being, then monitor responses and report condition changes to supervising nurses or physicians. Treat patients to relieve pain and restore function while instructing patients and families on care plans and progress. Visit patients according to the plan of treatment, complete progress notes per policy, and submit evaluations within state guidelines. Maintain maximum functioning regardless of patient background.
