Job Summary
Physical Therapist (PT) PRN for Home Health in Tennessee Quality Care. Provide PT services per physician plan of care, evaluate patient function, develop individualized care plans, educate patients and families, translate exercises into daily activities, modify equipment/home environments for safety, collaborate with physicians and care teams, document clinical notes and outcomes, supervise PT Assistants, and maintain accurate records while serving patients in their homes.
Required Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
- Current Physical Therapist (PT) license in the state of practice
- Current CPR and First Aid certification
- Reliable transportation, valid driver’s license, and current automobile insurance
