Physical Therapist - PRN - Skilled Nursing Facility

Conduct comprehensive evaluations of residents' physical abilities and needs, then develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function. Deliver therapeutic interventions including exercises, manual therapy, and training on assistive devices while monitoring progress to modify therapy plans as required. Collaborate with an interdisciplinary team to ensure holistic care and educate residents and families on therapy goals and strategies. Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory standards. Participate in quality improvement initiatives to contribute to the rehabilitation department's success.
