Job Summary
Conduct comprehensive evaluations of residents' physical abilities and needs to improve mobility, strength, and functional independence. Develop and implement personalized treatment plans, providing therapeutic interventions including exercises, manual therapy, and assistive device training. Monitor progress, modify therapy plans as needed, and collaborate with an interdisciplinary team to ensure holistic care. Educate residents and families on therapy goals and strategies while accurately documenting evaluations, progress notes, and discharge plans in compliance with standards. Participate in quality improvement initiatives. Requires current Colorado PT license, Doctorate or Master's degree, and experience in skilled nursing or rehabilitation. Flexible PRN scheduling available.
Required Qualifications
- Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
- Ability to work independently and adapt to a flexible PRN schedule
- Dedication to providing compassionate and effective therapy services
