Job Summary
PRN Physical Therapist to evaluate and treat residents in a skilled nursing/rehabilitation setting, develop personalized treatment plans to improve mobility, strength, balance, and functional independence; provide therapeutic interventions including exercises, manual therapy, and training on assistive devices; monitor progress and modify plans; collaborate with an interdisciplinary team; educate residents and families on therapy goals and strategies; accurately document evaluations, treatment sessions, progress notes, and discharge plans; participate in quality improvement initiatives within the rehabilitation department.
Required Qualifications
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
- Experience in skilled nursing or rehabilitation settings is preferred
