Job Summary
PRN Physical Therapist needed at Skyline Ridge Nursing & Rehabilitation Center to evaluate and treat residents to improve mobility, strength, balance, and functional independence. Provide evidence-based therapies including exercises, manual therapy, and training on assistive devices, while developing personalized treatment plans, monitoring progress, and adjusting care as needed. Work with an interdisciplinary team to ensure holistic, resident-centered care; educate residents and families on goals and strategies; document evaluations, treatment sessions, progress notes, and discharge plans; participate in quality improvement initiatives. Requires Doctorate or Bachelor’s/Master’s degree in Physical Therapy and current Colorado PT license; prior skilled nursing/rehabilitation experience preferred. Schedule is flexible PRN with pay between $52.00-$61.00 per hour. Location: La Junta, CO (US).
Required Qualifications
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
- Experience in skilled nursing or rehabilitation settings is preferred
- Strong communication and teamwork skills with a resident-centered approach
- Ability to work independently and adapt to a flexible PRN schedule
