Job Summary
PRN Physical Therapist to evaluate and treat residents in a skilled nursing setting. Responsibilities include performing comprehensive evaluations, developing and implementing personalized treatment plans to improve strength, mobility, balance, and overall function, providing therapeutic interventions such as exercises, manual therapy, and training on assistive devices, monitoring progress and modifying plans as needed, collaborating with an interdisciplinary team, educating residents and families about goals and exercises, and documenting evaluations, sessions, progress notes, and discharge plans in accordance with regulatory and facility standards. The role emphasizes resident-centered care, independent practice in a PRN schedule, and participation in quality improvement within a trusted senior living community.
Required Qualifications
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
- Experience in skilled nursing or rehabilitation settings is preferred
- Strong communication and teamwork skills with a resident-centered approach
- Ability to work independently and adapt to a flexible PRN schedule
- Dedication to providing compassionate and effective therapy services
