Physical Therapist - PRN - Skilled Nursing Facility

Conduct comprehensive evaluations of residents' physical abilities and needs to improve mobility, strength, and functional independence. Develop and implement personalized treatment plans including exercises, manual therapy, and assistive device training. Monitor progress, modify therapy plans, and accurately document evaluations, session notes, and discharge plans in compliance with regulatory standards. Collaborate with an interdisciplinary team to deliver holistic care and educate residents and families on therapy goals. Participate in quality improvement initiatives to support the rehabilitation department's success.
