Job Summary
PRN Physical Therapist to evaluate and treat residents in a skilled nursing facility; perform comprehensive evaluations of residents’ physical abilities; develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function; provide therapeutic interventions including exercises, manual therapy, and training on assistive devices; monitor residents’ progress and modify therapy plans as needed; collaborate with an interdisciplinary team to ensure holistic care; educate residents and their families on therapy goals, exercises, and strategies for continued improvement; accurately document evaluations, treatment sessions, progress notes, and discharge plans; participate in quality improvement initiatives within the rehabilitation department; license in Colorado; flexible PRN scheduling.
Required Qualifications
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
