Job Summary
PRN Physical Therapist (PT) to evaluate and treat residents in a Skilled Nursing Facility setting. Responsibilities include comprehensive evaluations, developing and implementing personalized treatment plans to improve mobility, strength, balance, and function; providing therapeutic interventions (exercises, manual therapy, training on assistive devices); monitoring progress and modifying plans; collaborating with an interdisciplinary team; educating residents and families; documenting evaluations, sessions, progress notes, and discharge plans; participating in quality improvement. Requires Doctorate or Bachelor’s/Master’s in Physical Therapy and Colorado license; experience in skilled nursing or rehab settings preferred. Pay $52.00-$61.00/hour with flexible PRN scheduling in Poncha Springs, CO.
Required Qualifications
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
- Current and valid Physical Therapist license in the state of Colorado
- Experience in skilled nursing or rehabilitation settings preferred
- Strong communication and teamwork skills with a resident-centered approach
- Ability to work independently and adapt to a flexible PRN schedule
- Dedication to providing compassionate and effective therapy services
