Physical Therapist (PRN PT)

Job Summary
Evaluate and treat residents in subacute and long-term care settings with a variety of orthopedic, neurological, and post-surgical conditions. Develop personalized treatment plans to improve strength, balance, gait, and functional mobility. Collaborate closely with physicians, nurses, occupational therapists, and speech therapists. Document patient progress accurately and in a timely manner using our electronic medical records system. Participate in care plan meetings and family conferences. Provide compassionate, patient-centered care in a supportive team environment. Supervise Physical Therapist Assistants (PTAs) as needed. Report directly to the Director of Rehabilitation.
Required Qualifications
- Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
- New grads welcome! We provide excellent mentorship and training
- Previous experience in SNF, long-term care, or geriatrics is a plus
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of an interdisciplinary team
- Compassionate, positive attitude with a genuine desire to make a difference in residents’ lives
