Job Summary
Implement residents' personal care plans while supporting their physical, spiritual, emotional, and clinical needs across multiple diagnostic groups including urinary incontinence, vestibular rehab, home safety assessment, and movement disorders. Complete documentation to meet reimbursement and regulatory requirements. Provide a holistic approach focusing on residents' strengths, preferences, and routines to ensure comfort and belonging. Work with the interdisciplinary clinical team to address care needs throughout the continuum. Valid state Physical Therapy license and Medicare-compliant training required. On-site physicians group and medical centers available for residents and employees.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
