Job Summary
Implement residents' personal care plans with a holistic approach to their physical, spiritual, emotional, and clinical needs. Focus on residents' strengths, preferences, and routines while providing comfort and companionship. Complete documentation to meet reimbursement and regulatory requirements. Prioritize therapeutic experience in older adults across multiple diagnostic groups. Work within an interdisciplinary clinical team to address care needs throughout the continuum, utilizing onsite medical centers for wellness and sick care. No additional travel required as all clients reside on the award-winning campus.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
