Job Summary
Implement residents' personal care plans while supporting their physical, spiritual, emotional, and clinical needs. Focus on residents' strengths, preferences, and routines to provide comfort, companionship, and belonging. Deliver therapeutic services across multiple diagnostic groups, including urinary incontinence, vestibular rehab, home safety assessment, and movement disorders. Complete documentation to meet reimbursement and regulatory requirements. Work onsite with no additional travel required.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
