Job Summary
Provide coordinated physical therapy to clients of all ages in home-health settings, planning, implementing, and evaluating treatment based on medical referrals and therapy assessments. Requires two years of PT experience, a BS in Physical Therapy, and active PT licensure in applicable states with CPR certification. Role involves travel to patients’ homes in Weatherford, TX (and surrounding areas), lifting up to 50 lbs, and maintaining documentation (Plan of Care, progress notes) while communicating effectively with patients, families, and health-care team members; participate in team meetings and continuing education. PRN schedules, flexible hours, and potential for career growth are offered.
Required Qualifications
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
- Current state PT licensure in applicable states
- Current CPR certification
- Valid driver’s license in good standing and reliable transportation
