Job Summary
Administer physical therapy to patients in their residences based on physician orders and plans of care. Develop and evaluate treatment plans in partnership with patients, caregivers, and families while supervising Physical Therapy Assistants and Home Health Aides. Participate in patient counseling, caregiver education, and ongoing interdisciplinary assessments. Communicate with physicians and health care practitioners regarding current care plans and contribute to all phases of the QAPI program. Requires a current license, two years of experience, and 100% travel to patient homes.
Required Qualifications
- Currently licensed in the state(s) in which practicing
- Two (2) years experience
- CPR certified with American Heart Association or American Red Cross
- Must demonstrate good customer relations skills and a commitment to providing quality service
- Acceptance and ability to demonstrate support the core values and goals of Agency
- Ability to demonstrate flexibility, initiative, and work with minimal supervision
- Must have a current valid drivers license, auto liability insurance and reliable transportation
- 100% travel required
- Graduate from a physical therapy curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and Physical Therapy Association
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
- Must provide evidence of annual TB test and other state-required tests or examinations
- Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials
