Physical Therapist PRN - Home Health

Job Summary
Provide physical therapy services to home health patients per plan of care, including assessing muscle strength, mobility, gait, ROM and transfer capabilities; instruct patients, families and caregivers in therapeutic appliances; initiate therapy programs and supervise/educate other personnel; participate in quality assessments and prepare progress notes; requires licensure as an Ohio Physical Therapist, either a Bachelor's or Master's in physical therapy, current CPR, and a valid Ohio driver’s license; minimum 2 years’ home health experience preferred. The role travels locally throughout the Tipp City, Ohio area providing patient services in a Home Health setting; shift: PRN. Required Qualifications
- Bachelor’s degree or master’s in physical therapy
- Current license as a Physical Therapist from the State of Ohio
- Valid Ohio driver’s license and access to reliable transportation
- Current CPR certification
- Complete a minimum of twelve (12) hours of in-services per year
- Experience: Minimum 2 years of physical therapist experience with community / home health preferred
