Job Summary
Implement residents' personal care plans while supporting their physical, spiritual, emotional, and clinical needs. Focus on residents' strengths, preferences, and preferred routines to provide comfort, companionship, and belonging. Complete documentation to meet reimbursement and regulatory requirements. Work with the interdisciplinary clinical team to address resident needs throughout their continuum of care. Experience in older adults across multiple diagnostic groups is preferred. Join a skilled nursing setting within a 108-acre continuing care retirement community offering onsite physicians, medical centers, and wellness visits.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
